SMA School Picture Day – August 20!
Get ready, Eagles! School Picture Day is coming up on Wednesday, August 20th. Students will also receive their school ID immediately after their photo is taken.
Please remember:
> Students must be in full school uniform.
> Consider purchasing a photo package — a portion of the proceeds helps cover the cost of student IDs.

HOW TO ORDER: Please circle the package of your choice. Also circle any desired bonus add-ons. Bonus add-ons may be ordered only as part of an addition to a complete package. All prices include required sales tax.
Total your order in the scoreboard in the lower right-hand corner of this form and attach payment to order form. No order will be accepted without payment. Preferred payment is either cash, Visa, MasterCard or Discover, or money order made payable to Musemeche Photography. If paying with cash, please place exact amount in an envelope and attach to order form.
If for any reason you are not satisfied with your delivered package, please return the entire package on retake day. You will be re-photographed on retake day, and a new package will be printed for you at no additional charge.
